Please use the cancelation form below if you need to withdraw your child from art lessons.

*Cancelation of an art lessons account must be submitted on or before the 20th of the month prior to the month you want it to take effect on your child’s art lessons account.

If canceling more than two children’s art lessons accounts, please fill out and submit the appropriate number of cancelation forms for each child’s art lessons account. If cancelation form is submitted after the 20th of the month any payment(s) processed for the following month will not be credited or refunded in anyway.

Cancelation of your child’s art lessons account must be completed through the online cancelation form only and can not be completed over the phone, in the office or through email.
